The Trick to Downloading the New DWC Forms

Can’t download the new forms (5021, PR-2, PR-3 and PR-4) from the DWC’s website? We can help (for free!).

On October 1, 2015 ICD-10 introduced 69,823 new diagnosis codes to California’s work comp system–as well as 4 new forms for treating physicians. As of January 1, 2016, these four new forms are mandatory, but accessing and downloading them is a bit tricky.

The Division of Workers’ Compensation (DWC) website provides links to the following four forms:

  • Doctor's first report of occupational injury or illness - DLSR Form 5021 (formerly DLSR 5020)
  • Primary treating physician's progress report - DWC PR-2
  • Primary treating physician's permanent and stationary report - DWC PR-3
  • Primary treating physician's permanent and stationary report - DWC PR-4

Problem: DWC Form Downloading Doesn’t Always Work

If you try to download one of these forms from the DWC’s ICD-10 page or the DWC’s Forms page, you may encounter this message:
