On October 1, 2015 ICD-10 introduced 69,823 new diagnosis codes to California’s work comp system–as well as 4 new forms for treating physicians. As of January 1, 2016, these four new forms are mandatory, but accessing and downloading them is a bit tricky.
The Division of Workers’ Compensation (DWC) website provides links to the following four forms:
- Doctor's first report of occupational injury or illness - DLSR Form 5021 (formerly DLSR 5020)
- Primary treating physician's progress report - DWC PR-2
- Primary treating physician's permanent and stationary report - DWC PR-3
- Primary treating physician's permanent and stationary report - DWC PR-4
