Sally Beauty Holdings: Changes TPA and Claim Numbers

Sally Beauty Holdings no longer uses CorVel as its third-party administrator (TPA).

Effective November 15, 2019, Broadspire administers all claims for Sally Beauty Holdings. The change applies to all dates of service.

IMPORTANT ALERT: Broadspire will NOT use CorVel claim numbers. Instead, Broadspire is assigning new claim numbers to all existing injuries.

Billing for Sally Beauty Holdings

Providers treating employees for Sally Beauty Holdings must call Broadspire at (800) 800-7885 to obtain the newly assigned claim number.

Broadspire does not know the claim number assigned by CorVel to existing injuries. As a result, Broadspire is unable to match an old CorVel claim number to its new claim number. To obtain the new Broadspire claim number for an injured worker, Broadspire requires the injured worker’s name and date of injury or social security number.

Best Practice Billing Tip: When speaking with Broadspire, confirm that your records match with their records for the following critical injured worker demographic and injury information:

  1. Injured Worker
  1. Name
  2. Date of Birth
  3. Social Security Number
  1. Injury Start Date

To send bills electronically, use Availity Payer ID TP021.

To mail non-electronic bills, providers should submit to:

Providers must submit original bills for all dates of service to Broadspire. Providers must also submit all Second Review appeals to Broadspire, even if the original bill was sent to CorVel.
