daisyData: Slowest Major Comp Payers Are Still Pretty Fast

Last week, we applauded the payers in our system that reimbursed daisyBill providers in the fewest number of days, on average, in 2025.

Below, we take a look at some of the slower major payers: the insurers, Third-Party Administrators (TPAs), and self-employed insurers that took the longest to reimburse our providers.

As it turns out, the news isn’t bad.

Of the 25 largest payers in our system by bill volume, even the slowest paid within a reasonable timeframe. There are small payers in our system (generally self-insured municipal employers) that take unconscionably long to reimburse; however, these payers receive relatively few bills from our providers.

Bottom line: electronic billing (e-billing) has (mostly) solved the problem of providers waiting months for reimbursement. Even the last-place finisher among our top 25 paid in under 18 working days on average.

While some payers have work to do regarding poor Electronic Data Interchange (EDI) performance, providers nationwide can generally expect quick payment for e-bills. Overall, that’s a win.

Reimbursement Speed: Top 25 Payers

The table below lists our largest 25 non-network payers in order of the number of bills they received from daisyBill providers.

The table reflects payment data for all bill types (medical, pharmacy, facility, interpreter, copy, home health, and transportation), excluding California Medical-Legal bills, that our providers submitted to insurers, TPAs, and self-insured employers from January 1 through November 30, 2025.

All of the top 25 accept workers’ comp e-bills from providers. All average days to payment reflect working/business days.

For each payer, we list the payer type, billing method, number of bills our providers sent in 2025, average days to payment, and the EDI “Grade” daisyBill assigned the payer based on its performance in processing providers’ e-bills.

  • Collectively, the top 25 paid our providers in 8.8 working days on average (payers that took longer than 8.8 working days are displayed in bold in the table).

  • The slowest of the biggest, the Los Angeles County Metropolitan Transit Authority, paid in 17.9 working days on average. This is about 3 days over the legal deadline for e-bill payment in California, which is 15 working days. Note that this payer’s EDI Grade is a solid ‘F.’

  • Coming in second-to-worst, Tristar Risk Management paid in 13.3 working days. Like the LA County Metropolitan Transit Authority, Tristar’s EDI Grade is also an ‘F.’ Coincidence? We think not.

Additionally, Intercare, ESIS, Keenan & Associates, the New York State Insurance Fund, and Next Level Administrators all had double-digit average days to payment. These payers aren’t exactly slow; it’s just that e-billing has raised the standard to the point where single-digits should be the expectation, not the exception. We urge these payers to step it up!

These data are good news for providers and injured workers.

The days of backed-up Accounts Receivable and chasing paper through the mail for workers’ comp bills are in the rearview. This is work comp’s e-billing era, and these numbers are a sign that technology is successfully cutting through the administrative hassles that keep providers from treating injured workers.
