Medical-Legal services are among the highest-priced in California workers’ comp. But too often, physician evaluators hand a significant share of their reimbursements to Medical-Legal management companies, just to get paid.
One Medical-Legal practice decided to do things differently.
After leaving its outside management company and moving billing in-house with daisyBill, the practice has been paid at 100% of Medical-Legal Fee Schedule (MLFS) rates on every paid bill, with an average payment time of just under 14 calendar days (state law gives payers 60 days to pay Medical-Legal bills).
In just over two months, the practice has collected $18,186.30 on 14 bills.
Unlike daisyBill, which charges a small flat fee per bill, Medical-Legal management companies can reportedly take up to 40% of each reimbursement. At that rate, more than $7,200 of this practice’s revenue would already be gone, with no guarantee that invalid denials or adjustments would be disputed.
If your Medical-Legal reimbursement data don’t look like the data below, consider using daisyBill or finding a management company that does.
The practice first spoke with daisyBill on July 2, 2026, agreed to move forward the same day, and set its launch date for July 13. The practice sent its first e-bills within a month of launching, on August 4. By August 13, it had received its first payment.
From there, daisyBill automatically tracked every bill through each stage of the payment cycle, from submission and acknowledgment to payment and Explanation of Review (EOR).
Between August 4 and October 13, 2026, the practice submitted 18 Medical-Legal bills totaling $23,670.68 in charges at MLFS rates. The results so far:
The fastest payment came from the California State Compensation Insurance Fund, in just six days. The slowest came from Omaha National at 48 calendar days, which is still within the 60 calendar days state law allows for payment of Medical-Legal services.
The practice received full MLFS rates for the range of Medical-Legal services for which it received reimbursement, including Comprehensive and Follow-up Evaluations, Supplemental Evaluations, Medical-Legal Testimony, and medical record review.
The practice has received one bill denial so far from Gallagher Bassett for a missed appointment charge that, if properly documented, should be paid. Through daisyBill, practice staff can submit a Second Review appeal disputing the denial within seconds.
The table below shows every Medical-Legal bill the practice has submitted through daisyBill as of this writing.
It’s a clear picture of what Medical-Legal revenue management looks like when evaluators insist on full MLFS rates, keep track of every bill, and keep the revenue they earn.

daisyBill automatically tracks every bill and payer response through the entire bill life cycle, with automated ‘Task’ prompts to guide billing staff when it’s time to take action.
For this practice, daisyBill will monitor the three bills that remain pending as of this writing and alert staff when missed deadlines or other activity warrant a response.
daisyBill will continue to track all of these bills and keep staff informed, so none of them slip through the cracks.
Many Medical-Legal evaluators are comfortable with their management company arrangements. But on bills worth thousands of dollars each, it’s worth running the numbers to determine what percentage you’re giving up, and how long you’re waiting to get paid.
Just as important, what data does your management company share?
Can you see which payers are reimbursing correctly, and how quickly? When a payer adjusts or denies a bill, does your management company submit Second Review appeals or escalate to Independent Bill Review? If you don’t have clear, bill-by-bill answers to those questions, you may be losing significant revenue without knowing it.
The technology exists to bill efficiently and protect against unnecessary losses. If you’re giving up significant revenue, waiting interminably for reimbursement, or both, consider a change that could transform your bottom line.
